When you need this form

Councils, schools and sporting clubs need their courts and fields checked on a routine cycle, and again before a season opens or a tournament runs. Inspectors look for surface wear, seam lifting on synthetic turf, ponding, worn line marking, loose net posts and goal frames that are not anchored. Outdoor assets are where public liability exposure concentrates, because the users are often children and the public.

When someone is injured on a surface, the question is what the last inspection found and what was done about it. Photographs of the exact defect, dated and pinned to the ground, answer that far better than a tick sheet in a shed. Asset owners also use the trend across inspections to argue for resurfacing budgets with evidence behind them.

What good looks like

• Surface condition rated per zone: goalmouths, baselines, run-off areas.
• Synthetic issues noted specifically: infill depth, seam lifting, fibre wear.
• Line marking legibility and correct dimensions confirmed for the code played.
• Fixed equipment checked: net posts, goal anchoring, padding, fencing, rebound walls.
• Hazards photographed with a play-on, restrict or close decision recorded.
