When you need this form

Some findings cannot wait for the next scheduled visit. A blocked exit, a failed emergency light circuit, a switchboard running hot, a split limb over a playground: these get escalated the moment they are seen. Raising a critical defect notice on the spot puts the building owner on notice, records the interim control and starts the clock. Auditors look hard at how fast that happened.

Timing is the whole argument in a defect dispute. A phone photo emailed later has no site, no time and no chain. A live notice pins the defect to the asset, the location and the minute it was raised, then carries authorised sign-off through to rectification. That record is what supports a fire safety review or an insurance claim.

What good looks like

  • Defect stated in one line, with severity and immediate risk named.
  • Asset identifier and exact location, not just the building name.
  • Photo or thermal image showing the defect as found.
  • Interim control applied: isolated, barricaded, taken out of service.
  • Authorised sign-off with the time the owner was notified.

Critical defect notice, answered

Is this a PDF or Word template?

Neither. It is a live form: your crew opens it on their phones, ticks it through, attaches photos and signs on the glass. Every submission is timestamped, GPS-pinned and stored as proof.

What counts as a critical defect?

Anything putting people, the building or continuity of service at immediate risk: blocked egress, failed fire equipment, exposed live electrical parts, unsound trees or plant. It needs an interim control now, not at the next service.

Who signs off a critical defect notice?

The person authorised to accept the finding on site, usually the supervisor or facility manager, plus the technician who raised it. Recording both, with times, shows the escalation reached someone who could act.
