When you need this form
Hot work, roof access, electrical isolation, facade work, plant shutdowns: any contractor task that can hurt someone or take a building system offline runs under a permit. The permit sets the scope, the hazards, the controls and the isolations, then names who is authorised to do it. Building managers use it as the single control point for everyone working on site that day.
Permits fail at close-out. A paper permit gets signed at the start and never closed, so nobody knows if isolations were removed or the area was left safe.
