When you need this form

Every business that puts people on someone else's site will eventually file one of these. A cleaner falls on a stairwell. A guard is assaulted at a venue. A contractor damages a tenant's fit-out. A machine injures an operator. Recording what happened, who was hurt and who was notified is the base expectation in Australian workplaces, and it is the first record a client requests.

The value of the record shows up months later. Insurers, lawyers and auditors all want the same thing: contemporaneous detail. A live form captures the time you actually opened it, the photos as the scene stood, the witness accounts while memories are fresh and the approval that closed it out. A report typed up on Monday cannot do that.

What good looks like

  • Date, time and precise location, captured at the point of reporting.
  • Injured person, witnesses and supervisor named, with contact details.
  • Immediate response logged: first aid, emergency services, area made safe.
  • Notifications tracked: client, regulator where required, insurer, with times.
  • Root cause, corrective actions and a dated close-out approval.

Incident report, answered

Is this a PDF or Word template?

Neither. It is a live form: your crew opens it on their phones, ticks it through, attaches photos and signs on the glass. Every submission is timestamped, GPS-pinned and stored as proof.

What should an incident report include?

The date, time and location, who was involved, what happened, the immediate response taken, any notifications made, the investigation findings, corrective actions and a sign-off that closes the incident out.

How quickly should an incident be reported?

As soon as the site is safe, ideally before workers leave for the day. Serious incidents carry short notification windows to regulators and clients, so most businesses report on the day it happens.
