When you need this form

Shopping centre dock areas are the messiest and most dangerous part of the site. Compactors, balers, grease bins and retailer overflow sit metres from delivery trucks and, often, from a public thoroughfare. Servicing these areas needs a documented routine: isolation before clearing a jam, spill control, housekeeping around the chute and checks that retailers are using the right stream. Centres carry heavy public liability exposure here.

If someone slips in the dock or a retailer disputes a contamination charge, the centre manager goes looking for the last service record. A live form supplies it with photos, a time and the name of the person who did the check. Insurers treat a consistent, dated record of housekeeping far more seriously than a laminated schedule on the wall.
