When you need this form

Spills in a shopping centre become claims fast: a drink dropped near a food court, tracked-in rain at an entry, a leaking bin trail through a mall. Centre management expects a response time, a wet floor sign, a clean and a check that the floor is dry. Slip claims are argued on what the cleaner did in the first ten minutes, and how it was recorded.

Claims often land months later. By then nobody remembers the shift, but a timestamped record does: the time the spill was reported, the time signage went up, a photo of the cordon in place, and the dry-floor check that closed it. That sequence is what an insurer and a centre manager want, and a paper log rarely survives long enough to provide it.

What good looks like

• Spill location by tenancy, level and nearest entry, plus substance type.
• Time reported and time attended, so response time is measurable.
• Photo showing wet floor signage and cordon before cleaning starts.
• Cleaning method and products used, matched to the substance spilled.
• Dry-floor check confirmed and signage removed, then signed off.
