When you need this form

Storing, decanting, applying or disposing of hazardous chemicals triggers this document before anyone starts. Commercial cleaning rounds, washroom sanitising, pool plant dosing, kitchen degreasing, airside and data centre work. It records the method, the hazards, the controls chosen, and the fact that workers were consulted about them. Clients ask for it during tender and contractor onboarding, often before the first shift.

Having the document is only half of it. Auditors want proof it was used on that site, on that day, by the crew who signed it. A filled record carries the sign-on list, photos of labelled containers and the spill kit, and a location pin. A template sitting in a folder proves intent. A completed one proves practice.

What good looks like

• A current safety data sheet named for every chemical actually on site.
• Controls matched to each hazard: dilution rate, ventilation, PPE, exclusion zones.
• Every worker signed on before starting, and re-signed when the method changes.
• Photos of labelled containers, storage and the spill kit in place.
• Supervisor sign-off closing the job, timestamped and pinned to the site.
