When you need this form

Force used by a security officer, however minor, needs its own report before the shift ends. Removals, restraints, defending against an assault, assisting police. The account has to cover what happened beforehand, the response chosen, injuries, and who was notified. Licensing obligations, client contracts and any later police or insurance process all rely on that first record being accurate.

Memory degrades and paper reports get written the next day. Captured on the spot, the report holds the time, the location, the officer's own account, photos of injuries or damage, and witness details before they disperse. Clients and insurers treat a same-shift record with attached evidence very differently to a statement reconstructed a week later.

What good looks like

  • Sequence recorded in order: what preceded the incident, the response, the outcome.
  • Force type and duration described factually, without opinion or justification.
  • Injuries to any party photographed and described, with first aid given.
  • Witnesses named with contact details, plus any police event number.
  • Notifications logged: client, control room, management, each with times.

Use-of-force report, answered

Is this a PDF or Word template?

Neither. It is a live form: your crew opens it on their phones, ticks it through, attaches photos and signs on the glass. Every submission is timestamped, GPS-pinned and stored as proof.

When do you need to complete a use-of-force report?

Any time force is applied, including restraint, removal or defensive action, no matter how brief and whether or not anyone was injured. Complete it before the officer finishes the shift.

What should a use-of-force report include?

The full sequence of events, the force used and why, injuries and first aid, witnesses, notifications made, evidence such as photos or footage references, and the reviewing manager's close-out.
