When you need this form

Toolbox talks happen at the start of a shift, when a new hazard appears, after an incident, or when a client changes a site rule. Five minutes covering the day's risks, the controls and anything learned since last time. Consultation with workers is a core duty under Australian work health and safety practice, and the talk is how most contractors evidence it.

Attendance is the whole point, and attendance is what paper loses. Signed on a phone, each worker's name sits against the topic and the date, with photos of the hazard discussed and actions assigned to a person. Client audits and tender submissions ask for a run of these records, and the run has to be continuous.

What good looks like

• Date, site, supervisor and the specific topic, not just safety talk.
• Hazards discussed tied to the actual work happening that shift.
• Controls agreed and who is responsible for each one.
• Every attendee signed on, including subcontractors and casuals on site.
• Actions raised with owners and due dates, closed out later.
