When you need this form

Fuel on an apron, toner in a branch back office, hydraulic oil under a forklift: different sites, same obligation to contain the spill before it spreads to drains or soil. Airside work adds access and security constraints, warehouses add moving plant, and bank branches often mean after-hours response with no one else in the building. Environmental controls have to be recorded, not assumed.

Environmental incidents get investigated backwards. What was spilled, how much, where did it go, who was told, what stopped it reaching the drain. A live form captures those answers at the scene, with photos of the bunding and the disposal, plus the notification time. That record is the difference between a contained incident and an unexplained one.

What good looks like

• Substance identified with its safety data sheet referenced, plus estimated volume.
• Containment steps recorded in order: stop source, bund, absorb, isolate drains.
• Photos of the spill area before containment and after clean-up.
• Waste disposal method and destination logged, including contaminated absorbent material.
• Site contact and supervisor notified, with names and notification times.
