When you need this form

Church and hall floors span timber, carpet, vinyl and stone, and each surface has its own strip, seal, buff and refresh cycle. This form holds the approved policy, the activity schedule, who does what, and where the method sheets live. Sites running volunteers and hire groups need that written down. Insurers and child-safety expectations both lean on evidence that the property is maintained to a plan.

Word documents drift. Versions multiply, the schedule on the noticeboard stops matching the one in the office, and nobody can say who approved it. A live schedule carries the approval, the distribution list and the supporting evidence in one record. When a slip claim or an insurance review arrives, you produce the plan and the proof it was circulated.

What good looks like

• Every floor type mapped to its own method and frequency.
• Named responsibility per task: contractor, staff member or rostered volunteer.
• Approval recorded with the approver's name and the date it applied.
• Distribution list showing who received the schedule and when.
• Linked evidence: product sheets, slip-resistance notes, photos of completed cycles.
