When you need this form

Facility managers carry the performance of every supplier they engage. Audit them on a cycle, after an incident, or when a certified management system requires evidence of supplier control. This form sets the scope and criteria up front, then records findings, non-conformances and corrective actions. Certification bodies look for exactly this: proof that supplier oversight happens on a schedule, not when something goes wrong.

Surveillance audits test whether your system runs or just exists on paper. Filled in the field, each finding carries its evidence, its owner and its close-out date, and the trail is queryable rather than filed. When the auditor asks for last quarter's supplier audits, you produce them in the room instead of promising to email them.

• Audit scope, criteria and the documents reviewed named before findings start.
• Each finding classified: observation, minor non-conformance or major non-conformance.
• Objective evidence attached: photo, document reference or interview note.
• Corrective action, root cause and responsible person recorded per finding.
• Lead auditor sign-off, plus supplier acknowledgement of the findings.
