When you need this form

Facility management contracts run on scheduled internal audits: a quarterly cycle, a new site brought into scope, a fresh corrective action after an incident, or a rehearsal before an external surveillance visit. This form captures the criteria audited, the evidence sighted, the findings raised and the actions assigned. Auditors expect that trail, and clients increasingly ask to see it before they hand over a portfolio.

A Word checklist proves nothing about when it was completed or by whom. A live record timestamps each finding, attaches the photo of the actual plant room, the actual register, the actual sign-in board, and carries the auditor signature. Close-out actions stay linked to the finding that raised them, so the corrective loop is visible instead of asserted in a summary email.
