When you need this form

Something did not meet the standard. A cleaning spec missed, a supplier delivering the wrong part, an audit finding, a repeat complaint from the same tenant. The register is where each one is logged, assigned, actioned and verified. Running a certified quality system without it is not possible, and it is the first document sampled in a surveillance audit.

Verification is where spreadsheets fall over. Anyone can mark a row closed. A live register holds the evidence that the fix actually worked: the re-inspection photo, the retrained worker's acknowledgement, the revised procedure, the date it was checked and by whom. Clients reviewing your quality performance want to see closure rates and closure proof, not a column of green cells.

What good looks like

• Each non-conformance stated as what was required versus what happened.
• Source recorded: internal audit, client complaint, supplier issue or self-identified.
• Root cause separated from the symptom, so the fix targets the cause.
• Corrective action with a named owner, due date and current status.
• Verification evidence attached, then closed by someone other than the action owner.
