When you need this form

Events and venues need a running log of anything that touches emergency response: a full evacuation, a partial move, an alarm activation, a false alarm, a medical hold on a gate, or a fire panel isolation. Crowd control briefs are built on it. Venue operators and insurers ask for the log after any incident, and expect it filled at the time rather than reconstructed.

Timing is the whole argument after an evacuation. When was the alarm raised, when did the floor clear, when was the all clear given. Entries made on a phone as it happens carry their own clock, so the sequence holds up. Add fire equipment servicing status and sign-off and the log answers the debrief on its own.

What good looks like

• Each event timed: alarm raised, evacuation started, area cleared, all clear given.
• Trigger recorded, including false alarms and drills, not just real evacuations.
• Head counts and assembly point checks against expected staff and patrons.
• Fire equipment servicing status and any isolation noted against the entry.
• Authorised sign-off by the shift supervisor, with photos where relevant.
