When you need this form

Aged care rooms, bathrooms and dining areas produce spills that carry infection risk: blood, vomit, faeces and urine. Staff need a defined response every time, not improvisation, because residents are vulnerable and the same surfaces are touched constantly. Providers run approved environmental cleaning and infection prevention systems, and each response is expected to be recorded against the room, the shift and the person who handled it.

During an outbreak review, the question is always the same: show us what was done and when. A live record answers it without hunting through folders, because every response is already attached to a room, a time and a named staff member. Photos of the disinfected area and the disposal step turn a claim of compliance into evidence a quality assessor can accept.

What good looks like

• Spill type and room recorded, with the resident's privacy respected.
• PPE donned and doffed in order, confirmed by the responder.
• Disinfectant used, its contact time, and the surfaces treated.
• Waste bagged, labelled and tracked to the correct disposal stream.
• Post-clean photo plus a supervisor or clinical staff sign-off.
