When you need this form

Extinguishers, hose reels, emergency lighting, alarms and exit signs all run on a service and test cycle, and the log is what proves the cycle was kept. Technicians fill it during scheduled servicing; building staff fill it for the in-between checks. When the statement or readiness review comes around, this log is the spine of the evidence, because a missing test period is the gap an auditor lands on.

Wall tags fade, get painted over and go missing with the equipment. Logging live means the record survives the tag: each item keeps its own service history, the failed test carries a photo of the gauge or the dead lamp, and the technician signs on the device. Overdue items surface as a list instead of being discovered during an audit walk.
